This argument is a tempest in a tea cup.
If someone can afford the $200K for the treatment costs, flying to the UK to get treated is a very minor expense.

I have a friend with stage 4 prostate cancer that goes to Germany and to Turkey for treatment as required.

I guess you missed the part that says can not be imported to then be sold for commercial purposes. The reg says nothing about purchase being forbidden for personal use rather it specified that it must be for personal use only and nothing about it needing to be free for use only that it can’t be resold.
When it comes to a personalized medicine like L reselling doesn’t really fit as it is only a good treatment for the person it is made for. There might be some overlap benefit for a blood type and HLA matched patient but not very likely as most tumors are very different from each other.
Bottom line is that a 3 month supply can be paid for and imported and that means cryogenic storage will be needed. Gee, didn’t Walgreens just increase their cryogenic storage for servicing the East coast from their new Pennsylvania facility with cryo storage?; ). Best wishes.
