Trooper Thank you so much for the quick response, as I said even the pharmacist didn’t know what was going on. I was going to call Blue Cross blue shield tomorrow but then again thanks for the clarification.

Wait - BCBS won't cover V if you're AT RISK of developing CVD, but don't have it yet? I have multiple risk factors for getting it, but have fended it off so far - with the help of V, of course - if I hadn't been taking it since 2013 I might actually have it by now - for insurance not to cover it for prevention SUCKS.

So if I understand you correctly BCBS says:
TG>500 mg and NO Diabetes plus 2 CV risk OR TG>500 and NO established CV disease MEANS generic omega 3 esters (EPA plus DHA like Lovaza) possibly gen V and/or fibrates and or anything so long as it is cheaper than brand Vascepa....
So, apparently, we are fixing the elevated TG risk of pancreatitis only and not worrying about CV risks.
This will mean (if strictly applied) a very few patients indeed being diverted to generic options--not even worth the edict.
STRATEGY
Even if you have a bit of plaque (by ultrasound CT or abnormal ABI<0.9)in your carotid, aorta or lower extremity arterial system or ANY part of your vascular tree--you have ESTABLISHED Cv disease and qualify per the main R-IT indication and qualify for Vascepa(though they may Tier out Vascepa and substitute gen V unless your MD says DAW, in which case they will stiff you the bill (classic economic blackmail)...try that route, if BCBS want to play silly buggers--have your doctor get on a call with their medical reviewer--two times and they generally pass on the discussion and approve (my experience).
