Yes, this form is used to reinstate revoked entity status.
If a corporation fails to file annual report and pay the fee to the Secretary of the State their status gets revoked. That's an administrative/involuntary dissolution.

RVUE's status was not revoked. They voluntarily dissolved the Corporation after liquidation and sale of assets.
