I haven't had the chance to comb through the cited chapters on page two. But might this form be what we're looking for in terms of ability to reinstate? I think they use this form for reinstatement if less than 5 years dissolved. Revival (a more lengthy and pain in the ass process) I think is a different form for more than 5 years dissolved.
