Here is the paper. Where did it say that the tumor was removed totally?

The column that says "NRD" in the chart you quoted where every single patient who was still alive had no residue disease. You can also note graph B in figure 2 that shows the survival for NRD vs RD, and how that is clearly the largest factor in OS. Second largest factor was type of sarcoma.

Did you see how tumor lysate was prepared?

Yes. Not sure if it matters much but they did not use the freeze/thaw cycle that -L uses.
