I haven't received a denial (sometimes took a peer to peer conversation) as long as they met all the FDA approved indications.

I have received denials if they don't perfectly meet the FDA indications- i.e. "off label". That could be a Trig of 149 instead of 150.

Since so much use of V is off label, it still is a problem.

Also, just because they may approve it on label it still may be a tier 4 and be prohibitively expensive for the patient.
