Cardiomd is right. Having major issues with insurance coverage. Some ask for TG above 500 and flatly deny. Some MDs will give up rahther than going through TAR process. In California, we use specialty pharmacy like Foothill remedy drug pharmacy. 818 236 2500. They do better than regular pharmacy taking some burden off from doctors, practice. They must know how to deal with Ins. Co and go easy on copay. High copays is a big issues.
