Kiwi, do you agree for CABG patients, placebo arm could have a 4.3% event rate if JELIS used optimal statins like in R-I? With additional 2g EPA, you could knock the event rate further down to 4% or slightly below, right? That looks pretty good Vs 5.2% placebo group event rate in R-I. To conclude, your optimal statin hypothesis actually improves the odds of R-I success.

I maintain that lowering LDL from 178 to 90 on high dose , high intensity Statins , will have roughly the same effect ( Hs CRP etc will also be further lowered ) as these JELIS patients with LDL at 178 plus 2 gms serum EPA

If you are right ...everybody: RUN AWAY ... R-IT composite rate is app. 4.84%, so V is killing patients ...

OR

if adding 4 gms of EPA , active arm dose will reduced the CV events to 3.2% annually ( PTCA and CABG ) and to 3.26% annually in the prior MI cohort. ... RRR will be 51% / 49% ... since R-IT composite rate is app. 4.84%

So what you are saying is that adding the CABG and prior MIs you get 895 +1050 = 1945 enrollees...which is about 53% of the 3664 souls in the JELIS secondary prevention sub set..Averaging the risk in this group comes to 4.6% annual risk...Since we know, from the literature, the annual risk in the entire placebo group was 2.3%...(Scratching my head) The only way the risk for the entire group could be 2.3% if 53% of the group have 4.6% annual risk rate, would be if there was some kind of "Lazarus effect"...or at the very least no events in the remaining 47% of the group..
