In 99% of cases: Nothing. Your broker handles the back-end routing. You just wake up on Monday morning (Jan 19) and notice that the "Bid/Ask" is being driven by the London Main Market instead of the OTC market makers.

I am stunned that anybody is selling at $.22 when they can sell through their US broker at $1.71

I am not surprised that no long can figure out that they can buy on OTC and sell on LSE and make a huge profit (and can put a small percent back in NWBO to grow shares at the same time).
