ICER said a fair price would be between $3,000 per year — if the drug is not as effective as promised — to $8,400, if it is.

That makes my $15 a day, $5,475 a year estimate for 273 pricing right in the middle of the target zone.

I think Dr. M is looking at keeping it reasonable to maximize usage.

Also the idea that you can charge more in the USA might be over. President Trump put through a ruling that the USA must get the best price (although the bureaucracy said it coundn't be implemented until after the election, imagine that). Since we know Biden is for the little guy, he wouldn't reverse that decision now, would he?
