This part got to me in the report.

Sounds like they picked up the phone and said send me an oxygen sensor. Pick one off the shelf and we will start from there. LOL

Sure sounds like a hit and miss to me.

the "permit required" sign is for a work permit, not the kind of operations permit JBII needs. The work permit required would be to do a given job during a shift in the "confined space". Safety issues, etc...
