Here are the various Lovenox doses in the FDA label:

 hip/knee surgery         then 40mg qD for 3 weeks 
  
 VTE prevention after 
 abdominal surgery        40mg Qd for 7-10 days 
  
 VTE prevention during     
 acute illness            40mg qD for 6-11 days 
  
 VTE treatment            1mg/kg BID or 1.5mg/kg BID for 5-7 days 
  
 ACS                      1mg/kg BID for 2-8 days 
  
 STEMI                    30mg IV bolus, then 1mg/kg for 2-8 days
