Presumably given the short time most of them lived, they were the "worst of the worst". Alan was told he had no other options and to enroll in hospice care.

Also, they were testing out several dosing regimens and maturation agents and one was preferable to the rest in terms of efficacy, so not everyone got the optimal formulation.

Also, it was a first in man trial with significant delays for many of the patients from enrollment to treatment. There was a required latency period. In a cancer like stage 4 pancreatic cancer where critical ducts and vessels can be involved, 24 hours can mean the difference between life and death.

The trial showed safety, optimal dosing and maturation formula and although it was not designed to show efficacy, the actual vs expected survival was meaningful especially given all of the criteria that was required.

All of the patients in the Direct trial had gone through at least two to three other treatments (that had eventually failed) by the time they were enrolled in the trial. I don't think it's much of a stretch to state that these patients were the "worst of the worst"... they certainly were by no means newly diagnosed.
