The exceptions pathway to adequate and well controlled studies is not obvious and is something you and others conveniently like to avoid as if JAMA Oncology should have done the same. Alas they chose the virtuous path less trodden. Of course the snake would never venture there so he slithers elsewhere and, like the Pied Piper, gets the unsuspecting to follow; ). Best wishes.
