Perhaps they treated the first 7 patients of the initial contingent of 38 patients with DCVax-L and may have been alarmed by the vaccine's induced pseudo progression. They decided to enroll the next 12 patients but did not treat them and waited to see how the first 7 patients were doing. With time they discovered that the first 7 patients did okay and may have suspected that the pseudo progression was beneficial but they were still not 100% certain that the pseudo progression was not harmful.
They therefore repeated the whole process in that the next 7 patients received DCVax-L and the following 14 patients did not. They again found out that despite the pseudo progression, the second contingent of 7 patients did well.
At that point they were certain that early DCVax-L treatment was most likely beneficial and were also comfortable in vaccinating patients after progression.

I am not sure that this is exactly what happened but something similar to that is a reasonable scenario (IMO).
