You're right. I had looked too quickly and assumed that he was referencing the PFS endpoint. I just went back and looked and he indeed was referencing OS.

Why did the treatment arm do worse (on OS) than the 99 patient placebo arm?

You are absolutely correct. There is no slide comparing how the DCVax-L treatment arm did against their own control group.

One should note, the endpoints for these trials are NOT medians... the primary and secondary endpoints are of overall survival... again, not the medians, although they can assist in a comparison of arms. A lot of researchers don't think that medians capture the long tail of a trial (such as the DCVax-L trial) because the median usually occurs before the long tail is evident on the KM curve. So the fact that they are measuring the entirety of overall survival in this trial will mean that they really be looking for the survival probabilities at each time interval, as calculated by the number of subjects surviving divided by the number of patients at risk, as shown here.
