I used to see an odd type of behavior when I was using NFS. Not sure which MM they were clearing through. I would keep refreshing and it would say open order even though I thought it should have triggered. Then if I cancelled the order to change it all of a sudden it would say order Filled or Sold.
So you can try canceling your order and see if the same happens to you. You can always enter a similar one moments later.
