That $3 isn't 3 months of sales

It's the risk associated with approval -- going from de minimis to de possiblis.

The market had us at 90/10, or maybe 80/20, and now it has us at 60/40... maybe a little better (2/3rds?)

60% x 20 + 40% x 5 = $14

2/3rds x 20 + 1/3rd x 5 = $15

I think it's likely we get approval too but I wouldn't put it past the FDA to issue a CRL as a middle finger. They see the drug is selling off label so they might use that as justification.
