Despite working at Amazon I don't have specific numbers to help on this. However, it's roughly about 2-5%.
The customer review percentage though can vary dramatically and depends on things such as (in rough importance order)
* How many reviews does the product have currently? If it has none then the feedback rates is higher, but this decreases rapidly as the numbrer of reviews increase. Greater than 10 reviews and people don't see the value of adding their opinion...
... unless they really disagree with the prevailing sentiment of the review, especially if their's was a negative experience compared to a positive sentiment.
* How controversial or high profile the item is. The higher the profile then the more feedback it will get, thus outstripping its actual sales. Kindles, political biographies by controversial figures are all examples of this as the review forum essentially comes a debating space (much to the annoyance of those who want a balance view on the book)
* Also, whether or not a product has marketing reviews (it does happen) can impact the total number of reviews.
* Enrollment in Amazon's vine programme (reviews of free samples by top Amazon reveiwers) will also impact the total.
