Was it so mangled that even an MD in "Emergency Medicine", who might have no occasion to deal with the aspects of medicine dealt with by CTIX's efforts, should have known better?

S/A would presumably need to provide a 1099 to some entity. He would incorporate for the same reason any other "business" incorporates.
