I hate when the charts get jacked like that as well. When this kind of stuff happens I change from Candlestick to line charts until it is fixed.

could the 12.6 million shares have somehow registered as coming off the ask from the market? the initial filing was December 31st.

I imagine the form is just required once you've accumulated a certain percentage of the O/S but I'll admit I don't know much about how all of that works...
