There is no uncertainty. 45 mg is effective no question but has long term safety issue. Does the benefit outweighs the risk for third line patients? Yes! Does the benefit outweighs risk for first line patients? NO! If its not going for first line I see no point of doing another trial to reduce the risk of caridovascular issues.
