93% of US commercial lives now have insurance coverage for Botox migraine

But not that many docs use it because it kind of falls between specialties. Primary care docs don't use botox, and I'd guess only something like half of neurologists administer it, plus some pain-clinic docs and all headache clinics (which aren't that common). I know someone in the Boston area that had a tough time finding a doc that would administer it (eventually ended up at a pain-clinic).
