I recently spoke with a doctor that follows this space very closely - he reviews and summarizes trials for a well-known service. His comment on the successful fish oil trials was that each one that actually showed benefit seems to have shown a different benefit (reduction in sudden death, reduction in CV events, etc.). That makes him uneasy.

JELLIS trial really was a very different population than you would see in the US (just look at their HDL levels), apart from any genetic differences.

I guess the take-home lesson right now is that nobody is going to put much faith in a biomarker study any longer - they are going to want to see actual outcomes.

I personally would absolutely not take a high-dose EPA product until I found out what it did to DHA levels. DHA is important for optimal brain and retinal functioning, and there is some chance high-dose EPA would interfere with DHA utilization (although there is evidence of some limited amount of EPA->DHA conversion with normal dosing).
