Agreed; the PML fatality rate in the historical dataset can be expected to rise gradually as it approaches the underlying fatality rate of the condition. Whether the fatality rate in the historical dataset levels off at 40%, 35%, or 30% is hard to predict, but the number will surely be higher than the 21% figure reported in #msg-60115928.
