I don't know why, but Broadridge wants to keep the nature of that report a secret:

Ans: "Please provide your nine digit cusip"
I am asking for personal research purposes only and accordingly have no cusip#. I have no interest in fees charged, just the composition and nature of the report. If that is somehow proprietary information only available to issuers simply indicate that that is the case and I will seek the information elsewhere.
Ans: "Yes this information is and can only be provided to issuers"
Seems nuts to me.
Before bothering to do this I did a pretty thorough google search and all paths lead back to Broadridge. I couldn't find a better description or an example.
