Topamax is used only prophylactically for migraines. That means people are on it for many months or years

People used it for years doesn't mean it was approved for use for years of continuous use. You can look at Topamax clinical study design to know the drug wasn't approved for indefinite use. FDA made clear long term use of a drug meant to be indefinite. Topamax wasn't, Qnexa is. Clinical trial designs for these uses are different, and the burdens of proof are different as well.
