The only CEO I know of who meets your criteria is Warren Buffet of Berkshire Hathaway, however, he's made some major blunders too. When he does,he will come right out and say what he's done wrong and not make any excuses. He can afford to, and I guess that's one of the major differences between him and other CEO's trying to make their mark.
Other than Warren, I think you're pretty much out of luck. All these CEO's are afraid to admit what they've done wrong and hide behind excuses for their failure.
