It's called common sense. Think back to your school days and the old game of whispering a secret from one person to the next in a circle. By the time the original "secret" came full circle, it would be unrecognizable - and that's just in a small circle of schoolchildren. Now throw in fear, bias, greed, hidden agendas, legal exposure, ignorance, miscommunication, and then filter it through a third or fourth party (alleged journalists) who know nothing or little about the subject and whose government seeks to portray its country (Russia) in the best light and certain countries (U.S.) in a bad light (see the recent NY Times article re direction and control of the media in Russia), then give the "information" to a biased source with an agenda (lawyer), and see what you come up with.
