There are lots of generic medicines and supplies which are used in very high volume on daily basis in hospitals ( Normal saline in 100's of liters). Which tertiary care hospital, leave alone smaller hospitals,  have you seen manufacturing any of those . But they will jump on manufacturing something orders of magnitude more complex, and that thing has orphan drug status , and will be used handful of times. I will stick with logic 101.
