The reason is money. It's always money. Italy doesn't want to pay for V so they made the reimbursement criteria so strict that almost nobody will qualify. They get to "say" they approved this great new drug (because outright rejecting it would look very bad) but the reality is that they have made it clear they do not want to pay for it. Why? Prevention just isn't a priority. No budget manager wants to spend millions now to save 10s of millions in 5 years because their job depends on being "on budget" this quarter or this year. 
The people making the reimbursement decisions are actuaries, not physicians.
