If it was the case that the effect was small then for them to have effectively said there is a 5 in 10,000 chance that the reason the placebo groups brains had more volume loss was due to chance it would mean the effect would have had to have been incredibly consistent i.e every single dosed patient would have had the same divergence.
It could be non-relevant AD parts of the brain but when you consider it's from the same study that found significance between dosed and placebo for ADAS COG and CDR-SB and even ADL despite failing trended in the right direction (FDA now say this is needed as too hard to show in early AD) then it would seem more likely to be correlated with the brain aspects of AD.
