I'd be willing to bet that very few of current patients are taking V for TGs > 500, most of them are off label just like most of Lovaza's sales were off label for CVD. And 5% market penetration for the R-IT indication would be a miserable failure - it's going to be SOC for all major medical societies (already is for some) - 25% penetration is quite reasonable, might even be on the low side. Malpractice lawsuits are won or lost based on whether the doctor followed SOC rec's, so any cardio not prescribing V to his patients with existing CVD for secondary prevention will be at risk if their patient drops dead of an MI.
