I just did this.... You can do so from your IRA but I don't think you can do it from your 401k... In order to do so via 401k money is by rolling it over to an IRA... And the only way you'll be allowed to do this, is if it is funds that are from a previous employer... Otherwise you'll pay a penalty (same guidelines as if you pulled money out of a 401k early for any other reason- though by it going into an IRA, it's at least tax sheltered- you'll pay taxes on the money later in life when you withdraw it as income) unless you put it in a ROTH IRA (you'll pay taxes on it now, but no taxes later)... Then you have to pay taxes on it... Unless it is coming from a ROTH source. Contact your portfolio brokerage and fill out a form... I faxed them the paperwork back to get it back to them as fast as possible... They might have a small fee for processing the roll over (I paid something like $40) and I paid extra to have them express mail the money to the new institution your are sending it to... But before you do any of that, have an IRA account open already as you'll want the account info for the paper work... Then once the new institution gets it, they'll put it in the account automatically, and there is a 6 business day hold on the funds...
