Specifically, I found a section from approx 43 - 46 of interest.

There are so many drugs out there being investigated that it becomes inconceivable to do combination trials with every possible combination. There would not be enough patients to do this on. And it would be too costly. So it becomes important to figure out in advance which combination trials have most promise.
Pseudo progression is an issue. Some patients might be dropping out of trials who may have actually had opportunity to see a benefit.
Patients are living longer is great. Trials are taking longer. Early indicators of effectiveness need to be studied. Biomarkers play an important role.
