We would very likely never find out if the FDA refused to allow a registration except in the circumstances of a clinical hold of some sort.
Basically communications between sponsors and the FDA are confidential - that's why there are sometimes unpleasant surprises when we see briefing documents for the first time and find out (for example) that the FDA had given the sponsor advice on trial design that was not followed.
