DHSM - Looks like collaboration between Tim Connolly, a few market makers, connections at FINRA, and the DTC.

Tim Connolly have been omnipresent. Along with a few other South florida traders, including, but not limited to their friends at Oppenheimer, they sure gamed the system for gain. Both up and down.
