If they don't cover Ztandi on Zytiga treated, they wouldn't cover Zytiga on Ztandi treated either.

What is Ztandi? Is that some Zytiga/Xtandi combo pill? ; )

Sequencing: this goes both ways.

Based on the two drugs respective MOA, it's hard for me to see Zytiga having any effect at all on Xtandi failures.

The converse, however, might not be true because of the efficacy of Xtnadi against splice variants and also I could see Xtandi (but not Zytiga) being effective in some other cases because it also (purportedly at least) blocks androgen receptor nuclear translocation.
