Thurly, please correct me if I'm wrong, but my impression is that while PS flipping does occur in all the tumor cells, not all the PS is flipped from the inside to the outside. Thus the effectiveness of the cytotoxic agents Bavi is being paired with in causing more of the PS to be flipped in each cell, rendering Bavi's PS action to be more effective.
