voo - use the one that you get from your broker.

In some cases, it might be identical, in other cases it might not, have 'code' numbers on it, or other stuff.

And certainly your broker will tell you what options you have -- it will depend on the broker -- eg, by mail, fax, phone, online, etc.

And lastly, most importantly, if voting via mail -- use the address your broker provides you.

The disclaimer on your ballot you posted, about not being able to use some of those methods, are for the MASTER ballot going from the BROKER back to KCCLLC.
