As far as significant bleeding risks with DVT prophylaxis dosing, it is not used in spinal operations as mentioned in the WSJ story, rather SCDs are often utilized. Also, many episodes of bleeding can be traced to polypharmacy, including concomitant or recent use of aspirin, Plavix or NSAIDs along with either heparin or LMWH.
