The person who referred me is close to the leadership, I have noted, closer than I am anyway. She said if the attachment doesn't work, just type the info into the body of the email. Of course, she may have been wrong. Anyway that's what I did, as I couldn't seem to even save the form, and my email wouldn't accept the use of the form, only typed information.
