If I were to speculate, I would conjecture that:
- the 10mg dose is cleared out of the system much more quickly than higher dosages, thus allaying FDA concerns, or anticipating them.
- to get a 40 mg dose effect, you can do a 10mg 4x, at shorter intervals.
- in order to make this regimen more palatable to pts who might balk at being infused in a doctor's office quite often, you set up an easier delivery (sq or nas) that is in-and-out or that is self administered.
