mistakes...

just off the top of my head...

1) underestimating the amount of troops needed to SECURE Iraq after our invasion

2) sending our guys without proper equipment, especially body armour and armoured vehicles

3) wholesale releasing of Iraq military troops

4) setting too low of standards (ignoring the geneva convention) in the treatment of civilians, especially the situation at Abu Graib

5) not giving the Iraq's the reconstruction jobs

etc etc etc
