You are talking about a different phenomenon. "Program-survival bias" as Dew has been using assumes that a phase-3 trial is designed based on one or more successful phase-2 trials and says something about the success rate of the phase-3. It's just his cockamammy way of putting together a few big sounding words to describe a trivial phenomenon (ie, any prediction with incomplete information has a chance of failing).
