This, almost certainly, is in part because injectors are getting very sloppy in needle placement (plus more immunogenic vaccines). The number of cases has grown immensely over last 15 years - with a significant fraction being permanent. I just had this problem after a TDAP vaccine. Then a few weeks later, when i went in to get COVID vax, the pharmacist tried to inject at shoulder joint, which is EXPLICITLY not per recommendations for giving vaccines (recommendation is 2 or 3 fingers below joint to avoid joint capsule). He argued with me so i literally blocked my shoulder myself with my other hand and then made sure he swabbed the correct area and injected there.
