Ha - well, sometimes you have a bug or something that needs to be fixed, then you think you got it fixed and that ends up not being the case when widely deployed. There's all sort of scenarios with variations on that. This was "unexpected" because none of us experienced anything like that when tested and it wasn't something we thought we already had fixed.
