I brought the 8 pages sent to me and my TDA statement. I was rejected at my chase, so I went to another branch and they gave it to me within 10 minutes.

Not sure if you got this answered or not, but I brought with me the medallion page and proof of ownership in the shares. I had TD draft a letter that showed I owned the shares with my account number attached to it. Hope that helps.
