The Pfizer trial started recruiting on April 30, 2020 when the pandemic was just getting started. There was not a lot of Covid floating around. Pfizer reported results of the trial on Nov 9, 2020. I don't know what the cutoff date was but on Oct 1, 2020 the were a total of of 5,467 deaths in the U.S. reported since the start of the pandemic. The total number of cases was 295,938. Most of their recruiting was during the summer months when people were outside a lot. It makes sense there there were not going to be a lot of deaths in either the vaccine or the placebo arm.

No deaths were considered by the investigators to be related to the vaccine or placebo. No Covid-19–associated deaths were observed. No stopping rules were met during the reporting period.
