When was a Super 8-K filed?

If it was a merger they would file a Reg S-X - not a Super 8-K.

And EWLL would have 75 days to file The Reg S-X and it would have to have 2 years of audited financials for the private company.

A Super 8-K is for a shell or blank check company.
